The Balmis Expedition Carries Smallpox Vaccine Across Oceans

Orphan boys carried live vaccine across the Atlantic and Pacific.

On 30 November 1803, the vessel Maria Pita departed the port of A Coruna in northwestern Spain, embarking on an extraordinary global medical venture. Led by military physician Francisco Javier de Balmis, the Royal Philanthropic Vaccine Expedition aimed to introduce immunization against smallpox across the vast territories of the Spanish Empire. Smallpox had caused widespread devastation in the Americas since the sixteenth century, recurrently causing severe epidemics over the subsequent three hundred years. Earlier preventative methods, such as variolation using live smallpox matter, had achieved inconsistent results and occasionally worsened outbreaks.

The breakthrough underpinning the journey came from English physician Edward Jenner, who demonstrated a safer immunization technique using cowpox material in 1796 and published it in 1798. Inoculation with cowpox induced a mild reaction while providing effective protection against lethal smallpox. The practice spread rapidly through Europe, reaching Spain by late 1800. After one of his daughters contracted smallpox and amidst pleas for assistance from colonial authorities facing outbreaks abroad, King Charles IV authorized a public health mission to transport the new preventative treatment to overseas dominions.
